Trying to get a note from a chiropractor to show that my back is messed up and I can’t work. Need to apply for Disability Benefits. What are the prices to getting an evaluation done to show there is something wrong with my back? Thank you!

Depending on your insurance or lack thereof, could be between $20-40 for a normal adjustment. If there is imaging involved (X-Rays, etc.) The prices goes up from there. But yeah, expect to pay around $40. It really depends on your chiropractor too.
